Whether a needs assessment is considered direct care depends on the purpose of the assessment and should be considered on a case-by-case basis. Assessments relating to care planning, support needs, care packages, carer support or placement decisions may be considered direct care. However, assessments relating to benefits or entitlements, such as Blue Badge applications, are not considered direct care and DCR information must not be accessed for these purposes. If you are unsure, seek advice from your local DCR lead or Information Governance team.

Can one team member access the DCR and share information with colleagues?

Teams often discuss patients and service users jointly, and sharing relevant information during case discussions or multidisciplinary meetings is acceptable where everyone involved is entitled to that information for the purpose of providing direct care. However, every authorised user who needs to access information in the DCR should do so using their own individual account. Users must not allow colleagues to access the DCR using their login credentials, and DCR information must not be shared with individuals or teams who do not require it for direct care purposes. This helps maintain patient confidentiality, supports audit requirements and ensures compliance with information governance policies.

What if I can’t find expected information?

Check data source status in the Provenance/Source panel.
