The new pathway allows\u00a0clinicians involved in the treatment of diabetes patients to access more accurate data when needed.<\/p>\n<p>There are 30,000 patients with diabetes living in Dorset \u2013 which represents 4% of the county&#8217;s population.\u00a0 Supported by the Dorset Care Record, the pathway removes the need to re-enter data and allows information to be shared between trusts. In doing so, it helps improve engagement between health professionals from different disciplines and, ensuring that different places in the care system can be used to inform and improve the treatment of patients.<!--more--><\/p>\n<p>The new diabetes pathway also allows clinicians to factor in unrelated conditions into patients\u2019 treatment plan. Previously, clinicians had to use separate systems for paediatric and adult patients and information could not be accessed from one trust to the next. As such, information had to be re-entered, making it difficult to record trends in patient conditions and making reporting back particularly difficult.<\/p>\n<p>In due course, all staff with DCR access will be able to see the diabetes pathway but it will be used by the multi-disciplinary group of medical professionals involved in caring for patients with diabetes, including podiatrists within the DCR Partnership, firstly at Dorset County Hospital and then at University Hospitals Dorset and Dorset HealthCare.<\/p>\n<p>To help, we&#8217;ve set up a range of videos for clinicians covering adding progress notes, completing a clinical encounter form, a dietetic assessment, foot assessment and enrolling and deactivating a patient.
