
Dorset Council adult access team members are using the Dorset Care Record while carrying out “91 Day Reviews”.
These telephone reviews are undertaken 91 days after a person has started having reablement care at home following discharge from hospital. They are a type of welfare check to see if they are still living at home and managing around the house, and have not returned to hospital or moved into a care home.
A Dorset Council adult access service support officer said the calls were important as part of the evaluation to see whether reablement support was successful and is a government requirement.
“There is a new list to complete on Mosaic each day, and often myself and my admin colleague will use DCR if a person is not answering our calls to see if the reason is because they have returned to hospital.
“It is much easier now that DCR is linked to Mosaic and I don’t need to sign into the site separately,” said the spokesperson.
Single Sign On was introduced for Dorset Council DCR users in March and is expected to go live for Bournemouth Council and Poole Council DCR Mosaic users either later this month or early June.
The spokesperson added that assessment and support co-ordinator colleagues in her team were also using the DCR.
