Here’s a good example from Dorset Council showing how the Dorset Care Record helps social care staff on a daily basis: “This client has a number of health issues, so I was looking up which teams were involved with her and to check if she had been referred to the Memory Assessment Service. I found out that she was being seen in the diabetic foot clinic, she had visits from the District Nurses to dress her legs and that she had bloods taken to investigate her memory but no referral had been made to the memory assessment service or community mental health team as yet.
