How one North Dorset pharmacist is using the Dorset Care Record

Community pharmacy professionals have been able to use the Dorset Care Record since the middle of last year, supporting their integration in the county’s health and social care system.

The sector was the first outside the acute and community hospital and social care settings to be given access to the DCR portal and they play a crucial role in supporting patients to make the most of their medicines and manage long-term conditions.

Principal clinical scientist Tanya Hart says the Dorset Care Record is providing people with better quality interpretation of their blood test results than in the past.

Dr Hart, who has been working at University Hospitals Dorset for the past 17 years, says part of her role involved interpreting blood test results and deciding which “abnormal” ones need urgent action and which are satisfactory. Out of the digital dark ages and into the light – how the DCR is helping medicines management technicians in Poole

When Fiona White qualified four years ago as a medicines management technician, she was constantly ringing GP surgeries and asking staff to fax through often hand-written information.

Now she is a regular and consistent Dorset Care Record user and loves its easy access and intuitive nature.

Fiona has been working at Poole Hospital since 2015. The role complements the hospital pharmacists but is different. Fiona is involved in supplying medication to patients while they are in hospital, in preparation for their discharge; counselling patients on their past and current medication; carrying out technical checks and liaising with members of the multidisciplinary team to ensure a safe and efficient service.

Community information through the DCR providing a lifeline for bone specialists

Osteoporosis and Fracture Liaison Service (FLS) lead nurse practitioner at Yeovil District Hospital Clare Cockill is an enthusiastic user of the Dorset Care Record.

Clare, who has practised at Yeovil since 2006, said that in an average year Yeovil FLS deals with around 1500 new patients with fragility fractures, many of them elderly due to the demographic nature of the population in north Dorset and south Somerset.
