Dorset Council goes live with adult referrals in the Dorset Care Record

Adult referrals from Dorset Council (DC) have this week been added to the Dorset Care Record, expanding the amount of social care information available.

The move will be particularly beneficial to other health professionals, who will be able to see details of when older people, informal carers or younger adults with learning difficulties, visual impairment or physical impairment have been referred.

Community physical health data to be used in the Dorset Care Record

Community physical health data is to be used in in the Dorset Care Record (DCR) to help provide a better service for patients.

Physical health data includes information from community hospital inpatient stays, therapy services and district nursing alongside many other services that use SystmOne.

All the additions are in line with the DCR’s aim to bring all health details together so medical and social care staff can give the best possible care.

Greater integrated care across Dorset as more feeds from acute hospitals enter the Dorset Care Record

People living in Dorset can look forward to enhanced coordinated care as further records are joined up within the Dorset Care Record (DCR)

The next tranche of health and social care records came on stream this week. Allergy information from University Hospitals Dorset NHS Foundation Trust (formerly Royal Bournemouth and Christchurch Hospitals (RBCH) and Poole Hospital (PH) will be available to health and social care professionals using the DCR.

It will mean that allergy details from across all of the county’s acute hospitals will be in place. This will be useful for hospital teams who will be able to prescribe medications more safely to patients

University Hospitals Dorset have also added clinic letters into the DCR as well as electronic inpatient and emergency discharge summaries, which will provide clear advice to GPs and social care teams helping to provide ongoing care to patients.

FAQs for Community Pharmacies

As part of our ongoing engagement with community pharmacies, we have developed a Frequently Asked Questions sheet which is available on the Dorset Local Pharmaceutical Committee (LPC) website.

Dorset Care Record Stakeholder Update – July 2020

Over the next month, community and mental health records from Dorset HealthCare University NHS Trust (DHC) are being fed into the Dorset Care Record (DCR).

Demographic information, which includes the name, date of birth, address, next of kin, NHS number and ethnicity of Dorset’s residents and inpatient and outpatient (attendance) details held by Dorset HealthCare are the first records to go live.

Bournemouth Christchurch and Poole Council (BCP) goes live with Single Sign On for Dorset Care Record

One of our local authority partners, Bournemouth Christchurch and Poole Council (BCP), has today introduced Single Sign On for staff accessing Dorset Care Record (DCR) information.

All current DCR users who have access to Mosaic (adult services system) have – since earlier this spring – been able to send demographic information as an electronic feed into the DCR.

Now, they will be able to go straight into the DCR system at a touch of a button. Launching DCR with user and person context will save professionals time, with one click getting to the relevant record. It will ensure that people will have a safe and slick journey through the service and will only have to “tell their story once”. Community pharmacies to join the Dorset Care Record

Up to 150 community pharmacies across the county are to be given access to health and social care records held within the Dorset Care Record (DCR)

This will give them up to date records in a transparent and easy to understand way – particularly important in this current time.

Pharmacies are the first sector outside the acute and community hospital and social care settings to be given access to the DCR and the move has been fast-tracked due to the COVID-19 pandemic.

Dr Craig Wakeham – A DCR Appreciation

Our Working Group yesterday held a minute’s silence for Dr Craig Wakeham, the Chief Clinical Commissioning Officer at NHS Dorset Clinical Commissioning Group, who passed away on Saturday 18 April 2020, aged 59. Craig had tested positive for COVID-19.

We were lucky enough to have the pleasure of working with Craig over the lifetime of the DCR and for some of us, many more years. Craig was one of our Programme Board members and regularly met the team during monthly meetings which were often followed by CCIO sessions at Upton House, Poole.
