“As self-management coaches, we see people who often talk about the frustrations of different teams of healthcare professionals not being up to speed on the details of each others care and plans.
This is even more confusing for people when care is provided by a range of different organisations, and community organisations and charities too. Juggling all of this and keeping on top of it for appointments and having to repeat often upsetting stories is really hard for patients and their carers.
Once healthcare professionals become confident at using the care record, and patients can contribute to it, it will be easier for people to feel in control of their healthcare and be more confident about what they need to do themselves.”
