Community physical health data to come on line soon

Community physical health data is to be used in in the Dorset Care Record (DCR) to help provide a better service for patients.

Physical health data includes information from community hospital inpatient stays, therapy services and district nursing alongside many other services that use SystmOne.

All the additions are in line with the DCR’s aim to bring all health details together so medical and social care staff can give the best possible care.

The benefits include referrals being displayed in chronological order which will provide healthcare professionals with an idea of patient history as well as all documented notes from a community hospital current stay being provided. The data that will be visible to the viewer is:

  • encounters – a view showing a complete record for the patient during the occasion within that service
  • problems and issues – each module will display any items recorded during referrals
  • referrals – referrals in and discharges can be viewed to provide partners with an idea of patient history
  • observations – available to view in chronological order by date and saved time
  • sensitive and restricted records in SystmOne will be respected in the data view. This applies to whole records or specific data items that patients have chosen not to share
  • Looked after children and safeguarding units will not display within the data.

It will be improved and built upon further next year when data from mental health services will be added. Currently, data is shown for units with open referrals however development is underway to show closed referrals in the near future.

This is the first physical health data input from Dorset’s community hospitals and community teams. The information will be provided by SystmOne, software that Dorset HealthCare uses to provide medical data.
