Ended care spells changes marks greater understanding of patients across community settings

Clinical software firm TPP has made changes to its national estate allowing Dorset Care Record viewers to see a history of interactions between different services in community settings.

Until now, when a patient’s care spell is ended in a SystmOne community unit, the information was no longer shared and was not visible to clinicians using the DCR. For example, information around a patient who was under a physio team would no longer be shown when they were discharged. If they were previously admitted to Blandford Hospital, it wouldn’t show, but now it does.

The changes will benefit users who frequently look for community interactions, such as social care staff, who will now be able to see historic community spells. Continue reading “Ended care spells changes marks greater understanding of patients across community settings”

Diabetes Pathway added to the Dorset Care Record

We’re delighted to announce that our diabetes pathway is now available within the Dorset Care Record for staff at Dorset County Hospital NHS Foundation Trust. The new pathway allows clinicians involved in the treatment of diabetes patients to access more accurate data when needed.

There are 30,000 patients with diabetes living in Dorset – which represents 4% of the county’s population.  Supported by the Dorset Care Record, the pathway removes the need to re-enter data and allows information to be shared between trusts. In doing so, it helps improve engagement between health professionals from different disciplines and, ensuring that different places in the care system can be used to inform and improve the treatment of patients. Dorset Care Record Annual User Survey – Launched

To help us continually look to improve the Dorset Care Record (DCR) for professionals across our health and social care partnership we would be grateful if you could spend 5 minutes filling in our short survey.  It will allow you to highlight what you like and don’t like about the DCR and what you’ve love to see added or changed.

DCR: Functionality Update

The Summary View within the Dorset HealthCare HTML viewer has now been added to the Community view in the Dorset Care Record, enhancing practitioners information about patients from a mental health perspective.

Emily Gadd, DHC project team manager, said this will benefit all DCR users, so for example, those in an acute setting or in local authorities, such as social workers as it will allow professionals to see a summary of the most recent activities of DHC’s patients. Pine Pulling at RSPB Arne

The Dorset Care Record team has spent their volunteering day this year helping conserve one of our rarest heathland habitats. Equipped with gloves and tree clippers, the team spent the day  at RSPB Arne harvesting fast-growing pine trees that can spread rapidly through the lowland health and gorse.

The team saw a sika deer, which have become quite common on the reserve, but despite scouring the skies failed to spot the white tailed eagle which was seen at Arne at the weekend after flying over from the Isle of Wight. Continue reading “Pine Pulling at RSPB Arne”

Robots assisting the Dorset Care Record

All of Dorset GPs are being given click through access to the Dorset Care Record (DCR) thanks to an emerging form of business process automation technology based on software robots (bots) or artificial intelligence (AI) workers.

Telling the DCR Story – through Illustration

Talented artist Alex May has produced a Dorset Care Record (DCR) illustration helping tell our story in a visual way.

Alex, who works for Dorset Council  has produced a number of illustration across various local authority disciples but more recently has been concentrating on the council’s digital offering.

The DCR PMO approached him to see if he could produce a DCR illustration that could be used by communications leads to help highlight to partners the benefits of using the DCR.

Alex was briefed initially by our communications lead Tony McDougal and then came to listen to the thoughts of the PMO during one of our team meetings. We encouraged PMO members to say what resonated with them most about the DCR and it led to some of the images captured here.

We were keen to get across the idea of feeds coming into the DCR but also the two way nature of the partnership – that the partnership was not just acute health, but social workers and pharmacists and GPs

Alex has captured the myDCR (public portal) element too and the power of ownership – the innovative digital nature of the project, the strong relationships needed for this to succeed, the benefits of only having to tell your story once, the partnership work and the importance that this information is secure.

Removal of the allergies badge from the DCR

The allergies badge in the DCR Patient Banner is being temporarily removed tomorrow (21st April at 08:30) following clinical advice.  It will be reinstated at a later date once a solution is available to mitigate the clinical risk.

However, allergies can still be found in the following areas:

  • DCH and UHD allergies available to the DCR are displayed in the Problem List, on the Summary and in the Medicines Viewer’
  • GP allergies can be find in the Primary Care tab

Staff have been notified of these changes and sent slides earlier today (Tuesday 20 April)
