Ended care spells changes marks greater understanding of patients across community settings

Clinical software firm TPP has made changes to its national estate allowing Dorset Care Record viewers to see a history of interactions between different services in community settings.

Until now, when a patient’s care spell is ended in a SystmOne community unit, the information was no longer shared and was not visible to clinicians using the DCR. For example, information around a patient who was under a physio team would no longer be shown when they were discharged. If they were previously admitted to Blandford Hospital, it wouldn’t show, but now it does.

The changes will benefit users who frequently look for community interactions, such as social care staff, who will now be able to see historic community spells.

Martin Turner, Dorset Council Adult Services Locality Manager north, welcomed the changes: “We in Adult Services find DCR patient records particularly useful in respect of medical diagnoses/episodes and hospital attendance as it helps us understand progressive decline, the need for additional equipment and supporting the potential for continuing health care funding. And we find it useful with linking in with named practitioners to complete a full/generic assessment of need.”

Ian Haywood, Wessex Care Records Business Analyst, said the move by TPP effectively gives parity with the primary care interface by always showing the community interface.

Mr Haywood, who has been working with Dorset HealthCare in overseeing the changes, added: “From a clinician perspective, this fix does ensures a history of interactions with different services are shown and that’s hugely beneficial.”

TPP made the changes first to Dorset HealthCare’s community feed before rolling it out across the national estate.
