DCR: Our Journey so far….

We’ve updated our infographic which highlights the progress of the Dorset Care Record (DCR) as we enter a new decade.

The infographic provides a wealth of information about what is currently in the DCR, its current users and usage, numbers of records accessed and people who have decided to opt out and amount of engagement events that have taken place.

For example, we had 2,856 unique records viewed last month by more than 1,000 authorised and qualified health and care professionals following completion of training with 83 GP practices feeding data into the DCR.

We’ll be updating this again mid-year to show the ongoing progress taking place.

Dorset Care Record: A Review of the Year and look ahead to 2020

Tackling issues such as information sharing and sensitive patients by bringing together an agreed common approach among Dorset Care Record partners has been one of the year’s highlights for programme manager Elizabeth McEleney.

Elizabeth feels that this clear approach has helped overcome obstacles and helped build a strong foundation and greater confidence to help deliver a range of additional functionality for the DCR.

How to access your medical and care records

GENERAL INFORMATON

Individuals have the right to obtain a copy of their medical or care records, in accordance with the Data Protection Act 2018 (GDPR).This is known as a Subject Access Request (SAR) or right of access.

This request can be made in writing or verbally – most large organisations have a form that they prefer to be completed as it ensures they have all the correct information to complete the request.

A request should be responded to within one month, although in exceptional circumstances this can be extended to two months.

A fee cannot be charged for this service unless the request is excessive, manifestly unfounded or an individual repeats a request already completed. If a fee is charged it must be a ‘reasonable fee’ and the individual notified of this charge at the earliest opportunity.

A SAR can be refused if the request is manifestly unfounded or excessive.

Proof of identity may be requested prior to the SAR being undertaken.

An Issue of Social Justice – Sharing health and care records in Dorset

The Dorset Care Record is a good example of how shared care records become more effective as they mature. It has just gone life with a clinical maternity pathway for practitioners and will be considering implementing Orion Health Medicines next year. However, communications and engagement manager Tony McDougal told a recent King’s Fund event that there is more to do.

Large-scale digital care record projects can take time to find their feet. Yet, once they move from “seeing to doing” – from giving clinicians a “view” of information held in different IT systems to using it to actively improve care – they can take off rapidly.

The Dorset Care Record is just at that point. Having spent two years integrating hospital and GP information using the Orion Health Amadeus open platform, it has just gone live with its first pathway: a clinical maternity pathway that will support Dorset’s pioneering Better Births project.

DCR – Quicker access to better information

Occupational therapist Kelly James, who works for Bournemouth, Christchurch and Poole Council (BCP) in the Christchurch locality team, says the Dorset Care Record has helped provide her with quicker access to better information.

Kelly, who has been an OT in adult social care for the past two and a half years, said: “I started using the DCR as soon as it became available last year. I use it as part of information gathering for OT assessment. I use it every time I write a new assessment, which means I go there potentially on a daily basis to check medication and diagnosis.

“In addition, people who have dementia are not always aware of their diagnosis. If we call the GP surgery for confirmation for statutory services it can take a week for them to get back to us. This can delay the clinical assessment and our help for the individual. The DCR helps speed this up.”

Helping mums to be in Dorset – Our new clinical maternity pathway goes live for midwives and health visitors

In our latest blog Ellie Venton, Dorset CCG’s programme lead for Maternity Transformation/Better Births System Integration writes about how our new clinical maternity pathway will help improve the care and support for mum, baby and the family.

At Dorset LMS (Local Maternity System) we are very excited to be launching the Maternity pathway within the Dorset Care Record. The work was designed to support the new postnatal care pathway delivered as part of the NHS England national Better Births Early Adopter work.

The Dorset Care Record – Orion Health case study

Our tech provider Orion Health has produced a Dorset Care Record case study highlighting the challenge, solutions and results so far.

The case study, available on line and in print, talks about our phased approach to implementation, based on regular releases of additional data and functionality.

For those technically-minded, DCR is cloud-based, with hosting provided by Redcentric. Patient record matching is done through an interface between the Orion Health platform and NextGate’s market-leading Enterprise Master Patient Index (EMPI).

Reflections from Orion Health’s Customer Conference – Manchester

Delegates attending Orion Health’s annual conference in Manchester this week were given updates on precision driven health, medicines management, business intelligence and population health tools and heard about some of the latest regional information sharing.

Orion Health works across the world at 58 Health Information Exchange sites from 22 offices in 15 countries with 110m+ patient records. So, it was interesting to hear from Orion country leads about digital health work taking place as far afield as the United Arab Emirates, the United States, Canada, France, Spain and Australasia.

Better Informed means Better Care for Dorset residents – Further pathways added to the Dorset Care Record

People living in Dorset can look forward to enhanced coordinated care as further records are joined up within the Dorset Care Record (DCR).

The next tranche of health and social care records are coming on stream this week, which will mean that a quarter of the proposed data feeds within the Dorset Care Record have been successfully integrated.
